1. Introduction
Welcome to Unilab of Dade, Inc. These Terms of Service ("Terms") govern your access to and use of our e-commerce platform, which provides at-home diagnostic tests ("Tests"). By using our website or purchasing our Tests, you agree to these Terms. If you disagree, please refrain from using our services.
2. Services
Unilab of Dade, Inc. provides at-home diagnostic Tests, which are shipped to you for self-administered sample collection. You are responsible for collecting a capillary blood sample using the provided kit and returning it to our laboratory for analysis. Test results will be securely delivered to you upon completion of processing.
3. Eligibility
To use our services, you must be at least 18 years old and legally capable of entering into binding agreements. By purchasing a Test, you confirm that you meet these requirements.
4. Account Registration
To place an order, you may need to create an account. You agree to provide accurate, current, and complete information during registration and to update your details as necessary. You are solely responsible for maintaining the confidentiality of your account credentials and for all activities conducted under your account.
5. Orders and Payment
By placing an order, you agree to pay the stated price, including applicable taxes and shipping fees. Payments are processed through secure methods such as credit cards and Apple Pay. Prices and product availability are subject to change without notice. Orders are processed upon receipt of payment.
6. Shipping and Delivery
Tests are shipped to the address provided at checkout. We are not responsible for delays due to incorrect shipping details. Delivery times vary based on location and carrier.
7. Sample Collection and Return
Your test kit will include detailed instructions for collecting your capillary blood sample. It is your responsibility to follow these instructions carefully and return the sample within the specified timeframe. Failure to do so may result in invalid results or additional charges.
If the sample collected contains insufficient volume to perform all of the tests ordered, we will process the tests in the order in which they were selected. If the sample volume is too low to run any tests, you will be offered the option to purchase a replacement kit for $50 USD. Please note that refunds will not be issued if you decline this option.
8. Test Results
Your Test results will be securely delivered via email. While we strive for accuracy, we do not guarantee that results will be error-free or conclusive. Our Tests are not a substitute for professional medical advice, diagnosis, or treatment. Consult a healthcare professional for interpretation and medical guidance.
9. Privacy and Data Protection
We are committed to safeguarding your privacy. Our Privacy Policy, which is incorporated into these Terms by reference, outlines how we collect, use, and protect your personal data. By using our services, you agree to the terms of our Privacy Policy.
10. Limitation of Liability
To the maximum extent permitted by law, Unilab of Dade, Inc. is not liable for indirect, incidental, consequential, or punitive damages, including lost profits, data loss, or any harm resulting from your use of our services.
11. Dispute Resolution
These Terms are governed by the laws of the State of Florida. Any disputes arising from these Terms or your use of our services shall be resolved exclusively in the state or federal courts of Miami-Dade County, Florida.
12. Changes to These Terms
We may update these Terms from time to time. Changes will be reflected in the "Effective Date" at the top of this page. Your continued use of our services after any modifications constitutes acceptance of the revised Terms.
13. Termination
