Return and Replacement Policy
1. Overview
Thank you for choosing Unilab of Dade, Inc. This Return and Refund Policy outlines the conditions under which we accept returns and issue refunds for our at-home diagnostic tests ("Tests"). By purchasing our Tests, you agree to the terms of this policy.
2. Return Policy
Due to the nature of our Tests, which involve the collection and processing of biological samples, we cannot accept returns of any used or opened Test kits. To ensure accuracy and safety, all returned items must be unopened and in their original, sealed packaging.
3. Refund Policy
3.1. Eligibility for Refunds
You may request a refund under the following conditions:
- Order Cancellation: You may cancel your order before the Test kit has been shipped. Refunds for canceled orders will be processed to the original payment method.
- Shipping Issues: If you receive a defective or damaged Test kit, please contact us within 7 days of receiving the kit. We may request photographic evidence of the issue. Refunds or replacements will be issued at our discretion.
- Order Errors: If there was an error in your order that was not caused by you (e.g., wrong item shipped), please contact us within 7 days of receiving the Test kit. We will arrange for a refund or replacement.
- Intake Form Requirement: All orders require a completed intake form before they can be processed and shipped. If your intake form has not been submitted, your order will be placed on hold until it is received. Unilab of Dade, Inc. is not responsible for processing delays resulting from an incomplete or missing intake form, and such delays do not qualify as grounds for a refund.
3.2. Non-Refundable Items
The following items are non-refundable:
- Opened or used Test kits.
- Kits returned without prior authorization or outside the specified return period.
4. Refund Process
To initiate a refund, please contact us at DTC@unilabhealth.com with the following information:
- Your order number.
- Reason for the refund request.
- Photographic evidence (if applicable) for damaged or defective items.
Once your request is received, we will review it and notify you of the next steps. If your request is approved, we will process the refund to your original payment method. Refund processing times may vary based on your payment provider.
5. Shipping Costs
Shipping costs for returns or exchanges are the responsibility of the customer, except in cases where we have made an error or provided a defective product. We will provide return shipping instructions if necessary.
6. Policy Updates
We may update this Return and Refund Policy periodically. Any changes will be reflected on this page, and the "Effective Date" will be updated accordingly. Your continued use of our services constitutes acceptance of the new policy.
